Completing your patient forms before your visit allows us to spend less time on paperwork and more time getting to know you and discussing your eye health.
Submitting your forms in advance helps speed up check-in, keeps your appointment running smoothly, and gives your eye doctor more time to focus on your vision and any questions you may have.

Focus Vision accepts VSP, Davis Vision, and Superior Vision (Versant) for routine vision care. Medical eye care appointments for conditions such as glaucoma, diabetic eye disease, and eye infections are billed through most major medical insurance plans. Please note that we do not accept Spectera.
